Fosfomycin
Fosfomycin, originally named phosphonomycin, was discovered in Spain in 1969. It is a phosphonic acid derivative, with an extremely low molecular weight, and shows almost no binding to proteins. Fosfomycin is a unique antibiotic that is chemically unrelated to any other known antibacterial agent.
Fosfomycin is a bactericidal antibiotic that interferes with cell wall synthesis in both Gram-positive and Gram-negative bacteria by inhibiting the initial step involving phosphoenolpyruvate synthetase. It inhibits the synthesis of peptidoglycan by blocking the formation of N-acetylmuramic acid.
Fosfomycin has a broad spectrum of activity against a wide range of Grampositive and Gram-negative bacteria. It is highly active against Gram-positive pathogens such as Staphylococcus aureus and Enterococcus, and against Gram-negative bacteria such as Pseudomonas aeruginosa and Klebsiella pneumoniae.There is no cross-resistance, fosfomycin can be administered in combination with a number of other antimicrobial agents.Its unique mechanism of action can provide a synergistic effect to other classes of antibiotics including beta-lactams, aminoglycosides, and fluoroquinolones.
